New Program in California: San Joaquin Peer Navigation

Welcome, San Joaquin Peer Navigation

We are pleased to announce that the San Joaquin Peer Navigation program opened on October 1, 2020!

This program was created in partnership with San Joaquin County Behavioral Health Services.

The San Joaquin Peer Navigation program is completely peer-run. The goal of the program is to connect members to a full range of services to ensure continuity of care outside of the program. Staff provide linkage to community-based resources and medical and psychiatric services, and provide support and education to family and significant others.

Who is Served: Adults, ages 18 and older, who have recently participated in San Joaquin County Behavioral Health Services’ 24-hour crisis response services, which includes the crisis unit, the crisis stabilization unit, psychiatric health facility (PHF), crisis residential facilities, and mobile crisis support team.

Hours of Operation: Monday to Friday, 8:30 a.m. to 5:00 p.m.
