This is a “whatever it takes” model of intensive and assertive community-based wraparound services designed to help 100 clients stabilize and avoid further episodes of homelessness, hospitalization and incarceration. Telecare’s IFSP program will consist of three multidisciplinary teams that include clinicians and professionals from psychiatry, nursing, substance abuse, peer support, housing, employment/education, and other relevant fields. Based on their areas of expertise, team members will collaborate and deliver integrated services, monitor progress toward goals, and adjust services over time to meet clients’ changing needs.

Telecare’s program in Santa Clara County is a 100-member high-fidelity Assertive Community Treatment (ACT) program for adults and older adults with severe and persistent mental illness (SPMI), co-occurring substance use issues, and /or other challenging and complex needs that increase their risk for poor treatment outcomes. Wraparound services and supports will not only include clinical interventions, such as multidisciplinary assessment and treatment, team-based case management, 24/7/365 crisis intervention, medication support, and integrated co-occurring substance abuse services, but also non-mental health support in such areas as community integration, housing, employment and education, transportation, and benefits.

Home First North FSP is Orange County’s Housing Full Service Partnership (FSP) program focused on providing housing-supportive services and behavioral health services to individuals residing in Orange County. Our purpose is to support members in maintaining stable housing, including resolving housing challenges and providing case management and behavioral health-focused services. Our staff creates a supportive environment, including services that are based on individual needs, hopes, and dreams. Home First North FSP members have access to a multidisciplinary team that includes peer specialists, case managers, a psychiatrist, a nurse practitioner, masters-level team leads, and clinicians who have experience in providing housing-supportive services and behavioral health-focused services. Program serves: 1) Residents of Orange County, ages 18 and older, who have been diagnosed with a severe mental illness and 2) Individuals who have been housed and are currently at risk of losing housing in Orange County.

Home First Central provides intensive and comprehensive housing supports to individuals who are at risk of losing housing in the southern region of Orange County. Telecare will employ a range of housing focused case management and behavioral health interventions through a fully mobile triage and field-based case management approach. Staff will be deployed to provide services onsite at permanent supportive housing developments, scattered site apartments and other transitional housing locations where clients reside. Focus areas will include resident wellness checks; wellness groups, skill building activities, and housing stabilization focused case management services; behavioral health and substance use services, crisis interventions; conflict resolution and property manager mediation; and other interventions that focus on successful tenancy, eviction avoidance and community integration
