Welcome, Santa Clara IFSP!

We are pleased to announce that phase one of the Santa Clara Intensive Full Service Partnership (IFSP) program opened on November 18. 2019!

This program was opened in partnership with the County of Santa Clara Behavioral Health Services.

The Santa Clara IFSP provides intensive outpatient services to adults and older adults who have a serious mental illness and who have a co-occurring substance use condition. The goal of the program is to engage underserved and unserved members with wraparound community-based support.

Who is Served:

Adult residents of Santa Clara County, ages 26-59, who have been diagnosed with a serious mental illness, have a co-occurring substance use condition and are high utilizers of emergency services. These individuals may also be homeless or have been homeless for a total of 120 days during the last twelve (12) months.

Older adult residents of Santa Clara County, ages 60 and older, who have been diagnosed with a serious mental illness, have a co-occurring substance use condition and require intensive outpatient services.

Hours of Operation: Monday-Friday 8:30 a.m. to 5:00 p.m.
