New Program in San Jose: Santa Clara ACT

Welcome, Santa Clara ACT!
We are pleased to announce that phase one of the Santa Clara Assertive Community Treatment (ACT) program opened on November 18, 2019!
This program was opened in partnership with the County of Santa Clara Behavioral Health Services.
The Santa Clara ACT program presents a new level of care for the county, with the highest level of outpatient services to be provided. The goal of the program is to connect members with community-based services, as well as provide housing support and services.
Address: Santa Clara ACT is temporarily located at Partners in Wellness until a permanent site is secured and renovated.

Who is Served: Adults and older adults with severe and persistent mental health and/or co-occurring conditions that result in substantial functional impairments or symptoms.
Must have Medi-Cal benefits designated to Santa Clara County or be uninsured.
Must meet Medi-Cal medical necessity criteria.
Hours of Operation: Monday-Friday 8:30 a.m. to 5:00 p.m.
