About the Program: Sonoma ACT

Sonoma Assertive Community Treatment (ACT) supports people as they work toward recovery and the goals that matter most to them. The team includes peer specialists, personal service coordinators, and master’s-level social workers. Together, they provide person-centered support that builds hope, strength, and resilience.

Event Activities/Highlights

  • Celebrated Telecare’s 60th anniversary during the program’s fourth annual member picnic on July 15.

  • Hosted a “Summer of Love” celebration inspired by the 1960s.

  • Members and staff dressed for the theme and enjoyed time together.

  • Held a dance contest, an award ceremony, and a staff sing-along.

  • Shared food, fun, and connection during a favorite summer tradition for Sonoma ACT members and staff.

Continuing the Celebration
