To Make Referrals: Referrals come from San Joaquin County and are prioritized based the on level of need.
# of Members: 50 members
Who We Serve:
Adults with serious mental illness
Are at risk of acute or long-term locked facility placement and/or those who have had multiple failed transitions from locked facilities to lower levels of care
Hours of Operation: Monday - Friday: 8:30 a.m. to 5:00 p.m. Staff are available on-call 24 hours a day.
About the Program: The San Joaquin Connect I program is a Justice-Involved Full Service Partnership (FSP) program, where staff create a supportive environment, including wraparound services that are based on the needs, hopes, and dreams of the individuals we serve. We help individuals enhance their recovery process through activities that empower and assist them with stabilizing their health, and by partnering with community services to ensure continuity of care outside of the program.
