To Make Referrals: All referrals to Presidio Palms are prioritized and matched by the region’s Coordinated Entry System.

Who We Serve: Adults experiencing chronic or literal homelessness and have a disabling condition substantially limiting one or more major life activities (as defined by HUD Category 1).
Hours of Operation: Monday - Friday from 8:00 a.m. - 7:00 p.m.
About the Program: In partnership with San Diego Behavioral Health Services and San Diego Housing Commission, Telecare offers onsite supportive and case management services for persons residing at Presidio Palms.
Service participation is voluntary and not a requirement of residing at Presidio Palms. Telecare works with tenants to engage them in services according to their behavioral health and case management needs. Services are provided throughout the community based on preference: on-site in Presidio Palms PSH community space; in individual units; in the supportive service office; or in the community, if appropriate. These services focus on housing stability and retention, including but not limited to:
Outreach and engagement services addressing housing stabilization and case management needs.
Behavioral health assessment, screening, care coordination, and referrals for residents at high risk, in need of more intensive behavioral health services to maintain housing.
Addressing ongoing behavioral health care coordination needs.
Offering site-wide community integration opportunities (BBQs, celebrations, tenant education, and other groups).
