At the Telecare Recovery, Engagement, Navigation, Empowerment and Wellness (RENEW) program, you have the opportunity to connect with a group of people who are here to be your support team.
We believe that recovery starts from within and will do whatever it takes to support you on your recovery journey. We have a multidisciplinary team that includes a psychiatrist; a nurse; a program coordinator/licensed team lead; a clinician; a peer recovery coach; personal service coordinators with experience in drug, alcohol, and mental health treatment; and specialists in housing, education and employment, who are all here to help you on your path.
RENEW follows the Recovery-Centered Clinical System (RCCS) model, which drives the supportive environment and treatment. RENEW also provides wraparound services that are based on your needs, hopes, and dreams, and connects you to the proper resources that can help you achieve your goals.

To Make Referrals: Telecare RENEW N B Street serves adults ages 18 years and older who meet the specialty mental health criteria requirements defined by Sacramento County Behavioral Health Services’ quality management, policy, and procedures.
The program receives referrals from the county. Referred individuals may be uninsured, Medi-Cal beneficiaries, or privately insured .
Telecare RENEW N B Street is contracted through the County of Sacramento, Department of Health Services, and Division of Behavioral Health Services and funded by the Mental Health Services Act.

About the Program: At the Telecare RENEW N B Street program, we are here to support you as you develop the skills you will need to build a strong recovery foundation. Telecare RENEW N B Street is a program that will provide intensive outpatient services to individuals who are homeless, justice-involved, and have co-occurring substance use issues.
Telecare RENEW N B Street’s hallmark feature is a tiered pathway to recovery, with multidisciplinary staff organized into the following teams:
Outreach, Engagement, & Specialty Services Team provides peer support and specialty housing and employment services to all clients throughout their time in the program.
High Intensity/Risk Team stabilizes new clients, as well as any current clients in need of the highest level of wraparound support.
Moderate Intensity/Risk Team supports clients who have achieved initial stability and are ready to fully participate in groups and specialized vocational, educational, and employment services.
Lower Intensity/Risk Team supports clients who have established community tenure of one year or more, are meaningfully engaged in community activities and are preparing for a transition.
