60th Anniversary Party San Diego Programs Wrap Up
/About the Program: San Diego Programs
Six Telecare programs serving San Diego County celebrated together: La Luna ACT, Vida ACT, IHOT, CARE Court ACT, CARE Court Outreach, and San Diego AOT.
La Luna ACT provides multidisciplinary crisis support through clinicians, psychiatric prescribers, nursing staff, and peer specialists, emphasizing choice-making skills and harm-reduction techniques to help individuals stabilize symptoms and connect with community resources.
Vida ACT offers a multidisciplinary team that includes a nurse practitioner, a licensed team lead, a peer recovery coach, and case managers with specialties in housing, education, employment, and substance use, providing advocacy for individuals with criminal justice contacts.
IHOT (In-Home Outreach Team) provides community-based support services directly in individuals’ homes.
CARE Court ACT offers time-limited, community-based, holistic support to individuals who meet CARE criteria under California’s Community, Assistance, Recovery, and Empowerment Act, utilizing a person-centered approach to address complex needs.
CARE Court Outreach helps unhoused individuals with severe mental illness engage with and access support, reaching out to folks who might otherwise be unserved to create meaningful entry into care.
San Diego AOT provides court-ordered outpatient services to adults with serious mental illness under Laura’s Law through a multidisciplinary team anchored in recovery principles.

“I enrolled in school, worked hard, and eventually earned my Bachelor’s in Social Work, graduating Magna Cum Laude, and later my MSW — one of the proudest accomplishments of my life. During that time, Telecare continued to believe in me. To Telecare and everyone who believed in me — thank you. And to those who once looked down on me for being “just a Peer Support Specialist” — look at me now. I’m at the big table now — and I’m here to stay.”
