About the Program: Yuma ACT and Sierra Vista ACT

Telecare operates two Arizona Assertive Community Treatment (ACT) programs that celebrated together: Yuma ACT and Sierra Vista ACT. Both programs provide multidisciplinary wraparound services to ensure members’ success in the community, promote hope and wellness, and prevent unnecessary hospital readmissions. Treatment services are individualized and guided by members’ hopes and dreams, following Telecare's Recovery-Centered Clinical System (RCCS).

Celebrated Telecare's 60th anniversary at the Yuma office with both Yuma and Sierra Vista programs coming together!
The afternoon celebration welcomed stakeholders, staff members, and corporate visitors.
The event provided a wonderful opportunity for community celebration and support across both Arizona programs.
