To Make Referrals: Referrals come from San Joaquin County and are prioritized based on the level of need.

# of Members: 48 members

Who We Serve:

• Adults with serious mental illness

• Are disconnected from the mental health system, or are at high risk of returning to the hospital, or have struggled with transitions from the hospital back to the community

Hours of Operation: Monday - Friday: 8:30 a.m. to 5:00 p.m. Staff are available on-call 24 hours a day.

About the Program: At Telecare’s San Joaquin Connect III program, you have the opportunity to connect with a group of people who are here to be your support team.

We believe that recovery can happen, and we will do whatever it takes to help our members achieve their goals in life. We have a multidisciplinary team that consists of psychiatrists, nurses, clinical directors, clinicians/team leads, peer mentors, personal service coordinators, and a housing specialist, who are all here to help you on your path.

Our program is based on the Full Services Partnership (FSP) model, where staff create a supportive environment, including wraparound services that are based on your needs, hopes, and dreams. We also connect you to the proper resources that can help you achieve your goals.
