Telecare Los Angeles Hopes and Opportunities Program (LA HOP) operates under contract with the Los Angeles County Department of Mental Health to provide culturally competent mental health services to adults with serious mental illnesses. This program began in 1999 as one of the first Homeless Outreach Programs in the state of California. In 2007, LA HOP transformed into a Full Service Partnership under the Mental Health Services Act (MHSA).

Just the Basics

Members: 175
Population Served: Adults, ages 26-59, who; are at risk for hospitalization or institutionalization, are homeless or at risk of homelessness, face criminal justice involvement, have not been successful with traditional mental health programs, and those transitioning from institutional settings

Recovery Model

Telecare has created a recovery-centered experience for people served. Telecare's approach is based on a belief that recovery can
happen. By connecting to each individual's core self and trusting it to guide the way, it is possible to awaken the desire to embark on
the recovery journey.

Program Structure

LA HOP uses a multidisciplinary team which includes psychiatrists, nurses, a clinical director, team leader, personal service coordinators, and consumer providers. LA HOP offers a low staff-to-member ratio and provides the majority of services in the community.

We believe in a "whatever it takes" approach in supporting clients with finding their hopes and dreams.

Members have personal responsibility in their own recovery process and are empowered through their own choices with the support of staff.
Members actively participate in initial assessments and identify where they would like program assistance to achieve their goals.

Office Hours

  • Monday - Friday: 8:00 a.m. to 4:30 p.m.
  • Crisis services are offered 24 hours a day, 7 days a week 

Contacting A Family Member

Family members are contacted and involved in the treatment when it is desired and approved by the member.  An information release is completed and signed by the member allowing us to speak with a family member.
