Forensic Projects for Assistance in Transition from Homelessness (FPATH)
Telecare is partnering with the Washington State Health Care Authority to open King County FPATH, an evidence-based, recovery-oriented program based on a team approach aimed at minimizing the barrier of receiving services such as housing, transportation, and services for behavioral health conditions.

What We Do: FPATH is designed to provide outreach, engagement, and intensive case management to connect individuals with supports, housing, transportation, and health care services.
Teams within community behavioral health agencies will include enhanced certified peer counselors who have experience working with homeless populations and may have lived experience. They will build relationships with individuals and help connect them with supports including housing, transportation, and health care services.
Forensic PATH teams will coordinate with the other elements of the Trueblood Settlement to conduct outreach in partnership with existing PATH programs and local Homeless Continuum of Care organizations to link individuals to housing, services, treatment, and resources. Outreach may occur in a street outreach setting or in a service setting such as an emergency shelter, jail, encampment, or drop-in center.
Capacity: 180 members
Referral Process: The HCA provides a list of referrals. The PATH teams will conduct outreach and engagement to individuals on the list. Forensic PATH teams will conduct outreach in partnership with existing PATH programs, local Homeless Continuum of Care organizations to link individuals to housing, services, treatment, and resources.
Hours of Operation: FPATH is open 7-days-a-week: Monday - Friday 8:30 a.m. - 5:00 p.m. (weekends vary)
