Who We Serve: Adult residents of San Diego County who are currently on parole and are not medically fragile. Forensic history is reviewed as a part of the assessment process to determine appropriateness for the program.

About the Program: The CORE San Diego program is under contract with the State of California Department of Corrections and Rehabilitation (CDCR) to provide intensive case management services to individuals who are on parole, have a mental illness, and are at high risk for homelessness. CORE San Diego offers community outreach and engagement, case management, education and employment support, and benefits and housing assistance.
