About TMRS

Telecare manages one of Stanislaus County's Regional Service Teams, providing outpatient and intensive community support mental health services to an estimated 350 to 400 adults living in eastern Modesto.
A range of services are provided, from very brief interventions to long-term community treatment. Telecare works in close collaboration with the county to ensure these services are fully integrated into the Stanislaus Adult System of Care. Established in 1998, Telecare Modesto Recovery Services continually refines services to improve client and system outcomes.

Staff provide services to clients in a variety of environments -- in the office, the clients' homes, and the community. Telecare Modesto Recovery Services directly provides or links clients with resources that provide medically necessary behavioral health services that assist clients in living successfully in the community.

Population Served: Adults, ages 18 and older, who have been diagnosed with a serious mental illness.
Funded by: Arizona Department of Health Services/Division of Behavioral Health Services (ADHS/DBHS) and AHCCCS.

Recovery Model

Services are based on the philosophy that regardless of the degree of impairment, consumers of mental health services should determine what is important in their life. This results in a client-centered, individualized approach emphasizing personal choice and empowerment.

Services and Supports

Telecare Modesto Recovery Services provides or assists clients in obtaining a full range of services. These services can include:

  • Psychiatric assessment
  • Psychiatric treatment
  • 24-hour crisis response
  • Counseling and advocacy
  • Medication administration
  • Medication support
  • Medical problem evaluation
  • Substance abuse interventions
  • Assistance with entitlements
  • Assistance finding and keeping housing and employment or other meaningful activities
  • Support and education of family and significant others
  • Assistance in the development of peer relationships
  • Connection to self-help groups and community resources

Staff also provide support and consultation when clients are hospitalized and aid in their transition to the community upon discharge.
