The CHANGES Dual Recovery program serves individuals who are both dually diagnosed (with mental health and substance abuse diagnoses) and frequent users of emergency psychiatric care. The program serves 200 individuals, and acts as the Single Point of Responsibility (SPR) for these Alameda County clients. CHANGES is the first implementation of the innovative Dual Recovery treatment model developed by Telecare. The program integrates approaches demonstrated in the literature to be effective helping clients with dual diagnoses.

Treatment Practice and Philosophy
At the core of the CHANGES program is a focus on supporting clients in their process of recovery. CHANGES also uses stage-wise treatment, motivational interventions, harm reduction, personal strengths and data driven interventions, when working with clients and their families. Stage-Wise Treatment uses specific approaches that have been found to be effective for an individual's specific stage of readiness to change. There are five stages a person goes through when making significant changes in their lives. Motivational Interventions are designed to engage and motivate clients who have not yet found traditional interventions helpful. Harm Reduction is a set of strategies that helps clients reduce the harm done to themselves and their communities by their choices. Personal Strengths development is an approach that emphasizes working in partnership with clients, assessing strengths and personal planning choices that increase their personhood. Data Driven interventions work with clients using measures of the amount of harm and strengths in a client's life and providing feedback about the choices they make.

Program Structure
The CHANGES program integrates these philosophies into the program structure, which consists of two distinct elements: Outreach Services and the Center for Change. Outreach Services engage clients who have not yet recognized a need for help, offering motivation and information. The Center for Change provides support for clients in their process of recovery. The Service component of the Center provides services to clients using Intensive Case Management or an ACT-like (Assertive Community Treatment) team with 24/7 availability. Outpatient detoxification services are also available. The multidisciplinary staff consists of case managers, psychiatrists, nurses and consumer employees. The Self-Help component is designed for CHANGES clients or graduates who are in the maintenance stage, offering space and staff to help clients support each other in their process of recovery.
