San Mateo Transitions exists to provide intensive support to people who have a serious mental illness and who are also homeless or at risk of becoming homeless.

Subs Abuse Coord Nia Banks Takes a call while having lunch at her desk San Mateo Transitions was developed in partnership between Telecare Corporation and the San Mateo County Mental Health Services Division, and offers Assertive Community Treatment-like services to 180 individuals. Special populations: 40 also receive residential service, 50 are older adults, and 55 are criminal justice clients.

The program is funded through California Assembly Bill 2034, a homeless initiative, which allocated $50 million in 2001 to the development of innovative programs which would reduce homelessness, reduce jail days, reduce hospitalizations, and increase the quality of life for individuals with serious mental illness.

Accreditation
All of Telecare's ACT and Residential programs have received a three-year accreditation by CARF, the Rehabilitation Accreditation Commission. The accreditation extends until January 2005. Telecare also received exemplary conformance to CARF standards in four areas:
