When CARF visited the CHANGES program, they gave kudos to a couple of helpful tools the team was using to improve services. We've included those tools here so you can explore as you like, or put them into practice at your location.
Weekly Assignment Communication Tool
This tool is used by the Team Leader to assign caseloads by location. Caseloads rotate every two weeks, and each case manager completes this tool and shares it with the team to enhance effective communication of service coordination.
Co-Occurring Consultation Request Form
This form is used to help CHANGES work more closely with primary care providers. It allows primary care physicians to easily provide CHANGES staff with an update on health care services provided and ongoing coordination of health care needs of our members.
