About Forensic Outreach Team

At the Forensic Outreach Team (FOT) program, we are here to help you transition back into your community.

We know that the journey to recovery can be a tough one, and we believe that no  one should have to go through it alone. We have a full staff of peer specialists and  clinicians—many of whom have personal experience with mental illness, homelessness, incarceration, and drug and alcohol recovery—who are all here to help you on your path.

While you may be joining the FOT program because of a mandate, we want you to  know that, for us, it does not matter how you got to this point, what matters is that you feel better in your life. Before your release from jail, a team member will meet with you to facilitate psychoeducational groups where they will work with you on planning  your next steps once you have been discharged. 

We work with you to meet these needs by connecting you to a specialized program  that can help you with co-occurring treatment, life skills training, social security and Medi-Cal assistance, and community integration.

Recovery Model

Our services are designed using the Assertive Community Treatment (ACT) model to enhance the lives of individuals living with mental illness. Our staff offers advocacy for individuals who have extensive contacts with the criminal justice system and provides linkage to mental health and substance abuse programs.

Services and Supports

Services include:

  • In-reach jail groups
  • Outreach and engagement
  • Community-based, wraparound services
  • Linkage to a mental health and substance use programs

Office Hours

  • Monday - Friday: 8 a.m. to 4:30 p.m.
  • On-call staff availability 24 hours a day, 7 days a week

Referral Process

To become a member, you can be referred to us by the Los Angeles Department of Mental Health or give us a call at 562-864-7821.

Admission Criteria

  • Adults 18 years and older
  • Resident of Los Angeles County
  • Persons diagnosed with a severe mental illness
  • People who have been released from prison/jail
